PO Num: This is the unique purchase order number

Quote Num: This is the unique quotation number

Quote Date: This is the date when quote was created

Lead Source: This is how the lead was generated

Company: This is the lead company

Name: This is the name of the lead

Is Customer?: This describes if lead is an existing customer or not

Status: This defines if lead has been closed or not 

Rep: This is the sales representative name
